James Miller is an 86-year-old male client who was admitted to the local hospital one week ago for community acquired pneumonia and returned to the aged care facility yesterday. On account of care provided, James experienced a sudden fall in the blood sugar level which can be attributed to the medication error occurring as a part of care. The primary cause of medication error in this scenario is the advancing age of the patient. Undoubtedly, advancing age and associated changes significantly influence the drug metabolism, therapeutic effect and the ability of the excretory system to remove the excess drug from the body (Waring et al., 2017).

Another important not modifiable risk factors that contributed to medication error was the existing condition of diabetes. It has been reported that, patients with existing condition of Type 2 diabetes mellitus prescribed oral or intravenous antidiabetic drugs are at more risk of developing hypoglycaemia in comparison to individuals without existing condition of diabetes (Silbert et al., 2018). When the patient was hospitalised for management of pneumonia, James experienced unstable blood glucose level which led to modification of the drug treatment from oral antidiabetic drug to Lantus insulin 20 units. James was prescribed glipizide 10 mg and metformin 1000 mg once a day as oral antidiabetic drugs to manage the blood glucose level regularly.
