Introduction
Nurses are expected to make decisions during provision of patient care by using skills of critical thinking and clinical reasoning. Critical thinking is a broad term which comprises of reasoning about clinical issues including teamwork, collaboration and streamlining workflow. Whereas, clinical reasoning is defined as a complex cognitive process that requires formal and informal thinking strategies to assimilate and analyse patient information, evaluate the significance of the same and rule out alternatives. By using components of nursing process as a critical thinking model, the nursing professional plans and implements evidence-based practices, referred to as science of nursing seen as Caritas Processes.
Case scenario
This case study includes Sally Abraham, a 37-year-old single mother who was referred to the palliative care team due to metastatic cancer. It was first 10 years back, when she suspected a lump in her breast leading to diagnosis of CA breast. A biopsy was planned for definite diagnosis which revealed two more lumps in the breast. Sally is now receiving end-of-life care/terminal care comprising of symptomatic management. Sally complains of nausea, vomiting and generalised pain. Sally has a 14-year-old daughter assisting her during the health care and offering little sips of carrot juice.
On completion of the clinical assessment and after reviewing the medication regime, it was documented that Sally has been MS Contin 120 mg BD and morphine (Ordine) elixir 40 mg for breakthrough pain. However, there have been three episodes of breakthrough pain in the last 24 hours. Sally subjectively reports that the pain is constant and it is difficult to hold her pain. She also complains of bloating, small amounts of watery diarrhoea and vomiting. As a part of nursing care, short-term goals were formulated comprising of interventions that are family-centred.
Caritas processes
Jean Watson’s theory of human caring explains the actual language of caring in nursing. Caritas signifies cherishment and application of Jean Watson’s theory of human caring enables the nursing professionals to cherish all patients. The theory of human caring is concerned with the process of how nursing professionals express care to the patients. The theory emphasises the humanistic aspects of nursing intertwined with nursing practice and scientific knowledge. This nursing model states that nursing is concerned with promotion of health, prevention of illness, care of the sick patient and restoration of health. This theory not only emphasises the practises of health promotion but also allows treatment of diseases (Kathleen Sitzman, 2018).
According to Watson, the activity of caring is central to nursing practice and is much more effective in providing care than simple medical intervention. The theorist believed that a holistic approach to health care centralises the practice of caring in nursing (Watson & Woodward, 2020). According to Watson, the activity of caring can be demonstrated and practised by nursing professionals. This is because, the provided care promotes growth, excepting the individual as they are and setting reliable future goals (Watson & Woodward, 2020).
