Introduction

This essay discusses case of Mr. David, an Aboriginal Gadigal man, 55 years of age admitted to GMW due to acute abdominal pain, diagnosed with H. Pylori. David has been trying to manage his pain using paracetamol and ibuprofen at home to no relief. David is married to Yind Edward, knows English and has no known allergies. David has a plethora of medications. At the time of clinical handover, the nurse document certain abnormal findings as well as fluctuation in vital signs. Through this essay, polypharmacy will be defined followed by identification of drug-drug interactions in this scenario. Strategies to prevent polypharmacy will be discussed along with sources that can be used to identify the same.


Polypharmacy

According to the World Health Organisation, polypharmacy is defined as the concomitant use of five or more medication. It is a common phenomenon older people due to simultaneously existing several chronic conditions required in multiple medications to prevent and manage symptoms (World Health Organization, 2019). More than 2/3 of Australian population aged 75 years and above are administered five or more medications, including over the counter drugs and complementary medications (The Fourth Australian Atlas of Healthcare Variation, 2019). Polypharmacy may be necessary and unavoidable in certain individuals however it is a potentially risky phenomenon (Cantlay et al., 2016).
